Marblehead High School Athletics 2008-09
General Information
What your child needs to participate in athletics:
I- Academic eligibility is determined when report cards are issued to all students.
At that time, a student must be passing 20 credits including English and Wellness
Education. If a student has two failures, he/she is ineligible. Eligibility
is determined on the day all students receive report cards. For the fall season,
a student must be passing 20 credits during the marking period prior to the
season. First term grades determine eligibility for the winter season. Academics
must be a priority.
Lack of academic eligibility = no clearance
II. Present written proof from your physician stating that the athlete has
had a complete physical within the last 13 months. Students are ineligible
when the physical expires. Scheduling physicals should be planned early. If
your record is on file in the nurse's office, you must get another copy from
your physician for the Athletic Department. This must be taken care of prior
to the tryout/practice time.
No proof of physical = no clearance
III. An Athletic Department Permission Form signed by the parent or guardian
must be completed and on file in the Athletic Office before your student is
eligible to participate. Form cannot be signed by neighbors or family friends.
If you are going to be away, arrange to complete the form before you leave.
No permission form = no clearance
IV. User Fee must be paid. The one-time fee covers all sports, clubs, Fine Arts and activities, which have a paid coach or advisor. No student will be denied access into any program due to inability to pay the fee. Fees will be waived for those students on the Federal Free/Reduced Lunch Program. Arrangements may be made for partial payments, but a portion of the payment is due and payable when the permission form and physical is presented. Scholarships and installment plans are available.
High School student = $340.00
Family Maximum = $480.00
Checks should be made payable to "The Town of Marblehead".
The user fee is an obligation.
V. All other obligations such as the return of athletic equipment and/or uniforms
must be cleared at the end of the season or full restitution for replacement
must be made. Once a uniform and/or equipment has been issued, it becomes the
responsibility of the athlete.
Obligations not met = no clearance
WHEN EACH OF THESE OBLIGATIONS HAS BEEN MET, A CLEARANCE FORM IS COMPLETED BY THE ATHLETIC DIRECTOR AND/OR ATHLETIC DIRECTOR'S SECRETARY AND GIVEN TO THE ATHLETE. THE ATHLETE GIVES THE CLEARANCE FORM TO THE COACH AS PROOF OF ELIGIBILITY TO PARTICIPATE AND DRAW EQUIPMENT. THE ATHLETE CAN NOT TRY-OUT, PRACTICE OR PLAY WITHOUT A COMPLETED CLEARANCE FORM.
NO CLEARANCE = NO PARTICIPATION
All forms and checks are to be turned into the Athletic Office. No permission forms, physical forms or checks are to be given to the coach unless the athletic office is closed.
